He is trying to look downwards and to his left, but his left eye appears to divert abnormally and looks misaligned. This is called Strabismus. There is an adult’s thumb and forefinger gently pulling the boy’s eyelids open as the boy looks downwards. The boy finds this very uncomfortable and has a look of worry on his face.
